Continuing on new search results features and various changes and bug fixes.

Search result
- restart QAP after saving options when option for search result columns is changed
- display modified and created time as local time in search results (instead of UTC)
- when QAP window is maximized and showing stats, squeeze larger columns to show stats columns
- do not display stats columns if database disabled
- fix various display bugs in search results
Various
- add checkbox to Setup script to make Start menu icon creation optional
- display popularity index in all menus (not only in Frequent menus)
- replace Windows Pick Icon dialog box with custom Select icon dialog box
- fix bug when a shared menu settings file is not found
