There is a known issue on some (not all) systems with the
Windows Icon Picker: when the selected icon file path is longer than 74 characters, QAP can truncate the file name or simply crash after you hit the OK button.

The Icon Picker is a Windows component called by QAP when you click on the icon in the
Menu Options tab of the
Edit Favorite dialog box. It always worked since the first release of QAP (and Folders Popup, earlier). As a developper, I have no direct control on how this component works and I could not tell why it crashes on some systems when the path is too long. I don't have enough data to be more specific but this issue has been reported only on some Windows 10 releases.
Until I find a solution or until I code my own icon picker, you can bypass the Icon Picker by using the
Edit icon resource link to enter the path and file name of the icon file and the icon index, as explained in the
Edit icon resource dialog box.
