I have a case of an icon not showing up.
It's a little hard to explain (English is not my native language), so instead I show it in these two pictures. Here you can see that the icon for Sketchup does not appear on QAP. Strange when all the others are not missing.

The icon is stored in SketchUp.exe.
If adding the application in QAP --> Customize --> Add --> Application the icon will be displayed as expected.
Did you drag and drop these shortcuts from the Desktop and into QAP?
If so, edit the SketchUp favorite in QAP, choose the "Menu Options"-tab click "Change icon" (blue link) and browse to where SketchUp is installed and select SketchUp.exe.
That should solve the problem.

Ah, OK!
You can either add SketchUp as an application in QAP.
Or try the following (not sure if it works):
Right-click on the original shortcut (in the Multimedia folder?), choose "Properties", "Change Icon".
Does it point to something like: "%systemroot%\Install\{long string of letters and numbers}"?
If so, use the "Browse" button to browse to where the SketchUp.exe is actually installed, select it, and "OK" enough times to get out of there!

I've seen this before, and it is not a QAP problem as such. Sometimes during installation of applications the icon setting for shortcuts points to the Windows Installer Cache in C:\Windows\Installer\that-long-number. And, sometimes when moving shortcuts around the icon disappears. I have always solved it by changing that setting, or just recreate the shortcut from the actual folder of the exe.
